Responsibilities

• Follow the plan of care to provide safe, competent care/ services to the client.

• Help the client maintain good personal hygiene and maintaining a healthy safe environment.

• Provide assistance with bathing and grooming.

• Plan and prepare nutritious meals.

• Assist the client with ambulation as detailed by Care Plan.

• Perform routine housekeeping tasks.

• Encourage the Client's independence.

Qualifications

• Must have a high school diploma. 

• Must be 18 years or older.

• Must pass a physical

• Must comprehend the basics of personal care, housekeeping and meal preparation.

• Must be able to work some weekends.

• Must pass a background check. 

• Must have reliable transportation, current insurance and current driver's license.

RN

Responsibilities

• Perform admission, transfer, re-certification, resumptions of care, and discharge OASIS

        for the home care patient.

• Consult with the attending physician, concerning alterations of Patient Care Plans.

• Coordinate patient services.

• Participate in the development and periodic revision, of the physicians Plan of Treatment. 

• Evaluate the patient, family members, and home situation, to determine what health teaching will be required.

Qualifications

• Must be licensed RN in the State of Kansas.

• One or more years of home health is preferred.

• Must have a current CPR certification. 

• The ability to communicate, both verbally, and in writing. 

• Must have computer knowledge, use of a tablet as well.

• Must be able to pass a background check.

• Must have valid driver's license, current auto insurance and reliable transportation.
